What To Wear and Bring On Day One

Arrive 10 minutes before your appointment with your completed forms as well as your medical insurance cards, photo identification, medication list, and any pertinent medical reports.(if you were unable to complete your forms ahead of time and then you will need to come 30 minutes early).Expect your first appointment to take approximately 60-90 minutes. Subsequent visits typically range between 30-60 minutes.Wear comfortable clothes that allow for free movement.
